MSN, AGPCNP-BC
Therese Lange, MSN, AGPCNP-BC, is a board-certified Adult-Gerontology Primary Care Nurse Practitioner with experience across primary care, gastroenterology, integrative pain management, wellness care, psychiatry, hospice, and correctional health. She is passionate about providing compassionate, individualized care that supports both physical and overall well-being, with a strong focus on patient education, preventive health, and chronic disease management.
Teri has cared for patients in a variety of outpatient and specialty settings, managing complex acute and chronic conditions while developing personalized treatment plans tailored to each patient’s needs. Her clinical background includes digestive health, metabolic support, chronic pain management, wellness-focused care, and interdisciplinary coordination. She also has experience performing joint and trigger point injections, interpreting diagnostic testing, and helping patients navigate long-term health concerns through evidence-based treatment and ongoing support.
Known for her thoughtful and holistic approach, Teri strives to create a welcoming environment where patients feel comfortable, heard, and actively involved in their care. Her unique background in healthcare administration, wellness services, and massage therapy further strengthens her patient-centered philosophy and commitment to helping individuals achieve lasting health and improved quality of life.

Important Membership Information
T Wellness does not currently offer onsite specimen collection or CLIA-waived point-of-care testing. Laboratory testing may be referred to Labcorp or Quest, while imaging and other diagnostic services may be referred to Desert View Hospital or another appropriate facility. All third-party laboratory, testing, collection, kit, and service fees are separate from the monthly membership.
Appointments are scheduled through the T Wellness online scheduling system. Clinically necessary follow-ups may be coordinated during regular business hours, subject to availability. Membership does not guarantee same-day, next-day, or reserved appointment availability.
